The petitioner must disclose his avocation, annual income and other details as to whether they are an Income Tax assesse or not. If the petitioner is an I-T assesse, PAN card details must be furnished. The petitioner must declare if the information filed is based on his personal knowledge, and if not, the details of the source must be indicated. The petitioner should also state in the affidavit that no PIL on the same issue had been filed anywhere else.
